The whole mess started in the kickoff meeting where no one wanted to ask a 'dumb' question. Answers to those questions would have made a big difference.

Everyone nods their heads, seemingly agreeing but the next week when asked what we're trying to achieve, there's five different answers.

We fought fires 50% of the time and pushed out the deliverable timeline every few weeks.  

Two of six project contributors were consistently late in meeting commitments. The reason: their managers told them our project wasn't as important as their other work.
